It looks like the hornet's nest that is PDAC has been stirred up again. Every manufacturer of cervical collars must re-submit their L-0174 products for coding re-verification, and the end date that practitioners will be able to use L-0174 for those products at their current level of reimbursement is August 31st, 2011.
An industry insider believes that most of the cervical collars you have been using (Aspen TX, Vista TX, Miami J, CSI, etc...) will be down-coded to L-0172. What does this mean? If we look at New York State for example L-0174 currently reimburses at $232.00 L-0172 reimburses at $123.00. So New York State suppliers will be losing $109.00 in reimbursement whenever a cervical collar is applied. Another way to look at it is if you apply a cervical collar with an approximate cost of $40, you will make about $83 instead of $192. Why bother driving to the hospital and starting a chart?
Why is this coding re-verification happening? Our source blames Ossur. The rumor on the street is that Ossur recently submitted their new Miami J to PDAC and did not receive a favorable code. Ossur contested PDAC in regards to this. This appeal is what stirred up PDAC. So thanks to Ossur it appears as if the entire industry is going to be stripped of L-0174.
A June 10th 2011 NHIC newsletter reads, "HCPCS code L0174 (CERVICAL, COLLAR, SEMI-RIGID, THERMOPLASTIC FOAM, TWO PIECE WITH THORACIC EXTENTION) describes a cervical collar that is constructed of thermoplastic foam. It is a two piece system with posterior and anterior thoracic extensions. The thoracic extension must overlap the manubrium sternum anteriorly and to the thoracic vertebra T-1 posteriorly.
Products currently coded with L0174 which are listed on DMECS will be end dated as of August 31, 2011. A new coding application will need to be submitted for any product requesting this code. When the products are coded by the PDAC, those meeting the code specifications will be listed in the Product Classification Matrix on DMECS. Only those products listed may be billed using code L0174."
So what should you do? Prepare for the inevitable. It is likely that PDAC will mimic the same coding scenario they followed a few months back for L-0631. So replace your L-0174 cervical collars with L-0180 collars. In New York L-0180 reimburses at $343.00. PDAC isn't looking at L-0180.

It is a much more substantial brace than the typical "slap and strap" type of L-0174 neck brace.
