Knee flexion contracture of 135 degrees or less, which will yield slightly to passive stretching. (To approximately 120 degrees after passive stretching.) To assess pt, first massage all areas starting with the knee—around effected joint, then do SLOW GENTLE STRETCH just to the point of resistance. Hold there an allow pt time to relax and the body to respond to the slight pressure. NEVER OVEREXTEND A PERSON WITH CONTRACTURES DUE TO IMMOBILITY OR YOU WILL HURT THEM UNNECESSARILY, AND RUN THE RISK OF CAUSING PERMANENT DAMAGE.
Expected Outcome:
Enhanced ability to have proper hygiene. Increased degree of range of motion of knee. Prevent further deformities and related skin/tissue breakdown. Decreased pain and discomfort. Enhanced self-esteem and quality of life.
- Treatment per physician’s orders.
- Prepare patients joint by doing Neuro Stretch or thorough massage of the joint area and gently stretching just to point of resistance – NEVER OVERSTRETCH.
DO NOT ADJUST KNOB SETTING – SET BY THERAPY
Recommended method for therapy to set hinge degree: Unscrew both knobs to allow free movement. Slowly range knee JUST TO POINT OF RESISTANCE and tighten hinges. Make sure each side is of equal angles.
- Lay patient’s leg into the splint with the hinge slightly higher than the patient’s knee – it will work down slightly during the application.
- Fasten 2” black cuff straps, then the kneecap straps. Adjust the kneecap straps to exert a uniform tug (all straps even). These will hook across knee cap in an “X” or criss-cross.
- Run two fingers under each strap to ensure straps are not too tight.
- Don and doff per care plan instructions. Gradually increase wearing time as tolerated. Remove at least once per shift. Check skin and report any redness to appropriate person. Chart progress.
- To remove device, undo each Velcro closure on the outside of the patient’s leg and hook back on the strap to prevent snagging and to increase the life of the Velcro. While supporting the leg with one hand, remove the device from under the leg and store in a clean area.
- Position the patient in as appropriate body alignment as possible – as: with pillows between knees.
- When the device setting is no longer offering a mild stretch to the patient’s joint, contact therapy to readjust device to again provide a gentle stretch. Continue this readjustment process to achieve the most improvement in the patient’s range of motion. Remeasure every 30 days.
- Remove padding. Wash plastic with mild liquid soap.
- Close all Velcro on padding, hand wash or put into laundry bag and wash on gentle cycle of home-type washer in mild soap and cool water.
- NEVER USE CHLORINE PRODUCTS or a commercial dryer.
- Air dry.
